This form may be downloaded and printed. It is a waiver of liability for any consequences resulting from discovery of adverse information during the background investigation which may then be shared with the applicant's current employer.
To be considered for employment as a police officer with this Department, all applicants currently employed as police officers and who are applying for a police officer position must submit this completed and signed form at the time their application is submitted.
